Dr John Asare Adomoko was the anaesthetist during the latter part of an eye operation conducted at Mayday Hospital, Croydon on 4 January 1987. Within 45 minutes, the patient had died. An oxygen tube had become disconnected from the ventilator, which the defendant failed to notice for 6 minutes. By this time the patient had suffered a cardiac arrest and attempts to resuscitate were unsuccessful.

The defendant was convicted by a jury at the Crown Court of the crime of gross negligent manslaughter and sentenced to 6 months imprisonment, suspended for 12 months.
